Markup: creating PDFs

I need to know if it is possible to create a PDF automatically based on information from a form and pre-determined layout specifications.
If it is complex, I don't need a blow-by-blow. If someone could just point me in the right direction, such as a article, a book, etc, it would be greatly appreciated.

Yes, it is possible. Just enter "pdf on-the-fly" in Google and you will be swamped with possibilities.

You can also think about using a content management system such as Joomla which natively offers PDF creation. Might not be the best fit depending on what you're trying to do but might be worth checking out.
